Apply for the PA H.O.G. License Plate! (For Cars, Trucks and Motorhomes)

Harley-Davidson Motor Company and the Pennsylvania Department of Motor Vehicles have established the following guidelines for PA HOG® members who would like to purchase a special HOG® PA registration license plate: (1) Applicant must have a current membership in National HOG®; (2) Section "C" must not be filled in; (3) Mail the application to PA HOG® Plate; Jerry Longo; 1485 Fieldcrest Drive; Erie, PA 16505.

SPECIAL INSTRUCTIONS: (1) Complete all parts of Section "A"; (2) Complete all parts of Section "B"; (3) Do not fill in Section "C"; (4) Include your HOG® National membership number in Section "B", along with the name, address and phone number of your local H-D dealership; (5) Applicant must sign and date Section "D"; (6) Include payment of $30.00 in the form of a check or money order payable to: PA HOG® PLATE; (7) Mail application and $30.00 payment to Jerry Longo at above address.
Applications sent directly to the PA Department of Motor Vehicles will not be processed.
